Payment:
Deductible, copay or payments are required at time of service and is accepted as cash, check or credit card.

Patients are NOT allowed to carry a balance and MUST have a payment plan on file for ANY balance accruals
Private Pay:
We also accept private pay as an option for our patients. Please contact us for our rates. Prescriptions will continue to be reimbursed under the terms of your insurance pharmacy benefit plan. Please contact your insurance if you have any questions. For our out of network clients, we can provide you with a superbill that you will be able to submit to your insurance for reimbursement. Please note that Mountain View Health PLLC cannot determine how much your insurance will reimburse you.

Here at the Mountain View Health PLLC, we expect to see our patients more frequently when they are new or experiencing crisis or difficulties, please plan for this if you are paying out of pocket. We are not able to predict how many visits each patient will need prior to meeting them, as this is catered to our patients' specified treatment and health needs. Additionally, if you have difficulty maintaining your treatment plan, your provider will be able to refer you to another practice.
